Out of curiosity at what point does a parking penalty notice get issued
formally?
If it's not yet fixed to the car has it been issued and could the driver
refuse to accept it and drive off?

Where is this defined?

In the Bible. This is a Calvinist country. The Lord ordained it
from the foundation of the world.

Yes and yes, although the second yes would do the driver no good at all.
